Mahajanaadara Programme

• Issued to low income, needy , sick , disabled

• Application should be submitted to the Grama Niladhari of the area. Recommended to the Social Services Division with the recommendation of the Grama Niladhari

• If you are under60 years of age, a medical certificate is required when applying for illness

Destributing Equipment Programme (Wheelchairs/Handrails/Spectacles

• Being a low income earner

• Medical certificate for use of the relevant equipment.

• Grama Niladhari recommendation with the form

Student Aids Programme

• The scholarships are given to children who are currently studying in destitute families     where the father has died.

• A letter stating that the student is studying in the school.(With the signature of the Principal)

• Copy of Birth Certificate

• A death certificate or an affidavit if the father is deceased or abandoned.

Diseases Aids Programme

• Fishy,tuberculosis, funding for diseases such as Thalassemia.

• Medical Certificate of acceptance of the disease.

• Complete the form provided by the Divisional Secretariat.

• A certificate of income from the Grama Niladhari of the area

Disabled Housing Assistance Programme

• Complete the form provided by the Divisional Secretariat.

• The applicant should own a land (certified by a deed)

• If there is no land a person has the option of giving the land.

• Letter from Grama Niladhari to low income earners

• Certificate obtained from a recognized medical practitioner.

• Housing Plan and Estimate obtained from Technical Officers

• Birth certificate or a photocopy of the identity card to prove the age.

• Rs.Documents or any other document indicating how to obtain that amount for an estimate of more than 2,500,000

Livelihood Support for Disabled Persons (Rs.- 3000 Programme

• Medical report obtained from a Government Medical Officer on Disability.

• Recommendation of the Grama Niladhari

Single parent self-help Programme

• This assistance is given to women who have died or abandoned their husbandunder 50 years.

• Grama Niladhari certificate that low income persons.

• Death Certificate or Sworn Affidavit

• Priority is available if there are children of school age.

Disaster Management activities Programme

• Providing aids

• Inspection and recommendation for assistance.

• Providing reports

Vocational Training Programme  Programme

• Referral to vocational training for persons with disabilities.

• Focusing on self-employment
